Use this form if you received an email notification instructing you to do so.
Refer to the email notification's Fire Permit for Filming Information Sheet to fill in the questions below.
Additionally, please upload the following documents:
- Current Script
- Fire Permit for Filming Information Sheet (from the email notification)
- Dodge College Safety Plan
- Any SDS or other backup documentation as required by the Permit Request Form
Be advised that if your production includes a vehicle roll-in, the following rules apply. Electric and/or Hybrid vehicles are prohibited.
- The vehicle's fuel tank shall have no more than two gallons and gas tank cap shall be secured.
- The tank shall be empty at time of inspection; siphoning will not be permitted.
- The vehicle's batteries shall be disconnected and taped.
- The vehicle shall not be parked within the stage fire lane or block an exit.
- An oil drip pan shall be placed under the vehicle at all times.
- The vehicle shall have a dedicated fire extinguisher within reach.
- The vehicle shall not be operated at any time while in the building.
- The vehicle shall not be left in the soundstage overnight.
- No open flames or burning allowed (candles, matches, lighters, cigarettes, etc.).
- INCLUDE EMAIL from Vanessa Silva or Marita Tyler stating that she will be present to supervise.
